Sharing of pupil progress with the Key Stage team and headteacher

Classroom based assessment and monitoring

Data analysis

Following up parental concerns

Tracking individual children's progress over time

Information from previous schools and settings

Information from other services 

Under-taking, when necessary, a more in-depth individual assessment

Involving an external agency when necessary

Phonics Screening Assessment in Year One

Use of a 'POP' (Personalised Opportunity Plan) to track progress against specific targets
